New patients are welcome
- Contact reception
- Fill out Patient Registration Form/s in the surgery
- Book initial Registration Appointment with GP
- Once accepted, medical records can be requested from previous GP
New patients and families wishing to join the practice are invited to make contact with reception 071 91 23999.
Patients are welcomed to visit the surgery to complete a Patient Registration Form. Parents/Guardians fill in a form for each child under 16 years.
All new patients are invited to make an initial registration appointment with one of our doctors. This is a dedicated appointment which needs to be booked in advance so as to provide adequate time for our doctor to take your personal details and medical history.
Once accepted as a new patient, we can assist you in requesting your medical records from your previous GP.
If you have a medical card or GP Visit Card, you will be asked to sign an Application to Change GP form.
Visitors
Visitors are also welcome. If you are visiting Sligo from the EU and hold a valid European Health Insurance Card you do not have to pay for emergency GP visits or medication during your stay. Please remember to bring your valid card with you when seeing the doctor.
If you feel your condition is life threatening, you should call an ambulance by dialling 112 or 999 or go directly to the Emergency Department
